Hammertoes, claw toes, and bone spurs are the most common toe deformities and involve the lesser toes (2nd through 5th toes). With hammertoes, your toe is bent at the middle knuckle of the toe, while a claw toe involves the toe bent at both the middle knuckle and tip of the toe. A bone spur can happen anywhere on the toes but usually affects the 4th and 5th toes. Each of the deformities can create corns on the toes, and in more severe cases redness, swelling, and even an open sore can develop. You should be aware that corns are not a skin problem! They develop because the skin is being pressed on and aggravated between the bones in your toes and your shoe. Certain People Are More Prone to Hammertoe & Claw Toe

People born with a certain foot type are predisposed to hammertoes and claw toes and include those born with flat feet, high-arched feet, or really flexible feet. Other causes are extremely long toes, tendon imbalance, injury, rheumatoid arthritis, and neuromuscular disease. Over time, the mechanics of your foot and shoes increase the deformities. Women are more affected by these conditions than men, mainly because of the type of shoe they wear and the use of tight stockings.

Symptoms of Hammertoe & Claw Toe

How severe the deformity looks may not match the degree of pain. For instance, a hammertoe or claw toe that looks severe may have no pain; while deformities that look mild may be incapacitating.

Pain and corns usually grow over the toes due to the friction of the shoe in that area. This rubbing of the shoe may cause a red, inflamed sac of tissue called bursitis. In the beginning, the deformities are flexible and can be treated easily. However, if left untreated, they can become rigid or stiff and cannot be straightened easily. Over time, pain may develop deep in the toe joints and even the ball of the foot, limiting walking, exercise or even just standing comfortably. High heeled or pointed shoes and tight stockings can exacerbate these symptoms even more.

A bunion deformity involving the big toes can also result in hammertoes and claw toes – or make them worse. When the big toes start to angle over, the big toe may cause increased pressure on the second toe or the second toe may be pushed upwards out of the way. 

Treating Hammertoe & Claw Toe

When the pain from hammertoes and claw toes is mild or moderate, the simplest solution to try is a wider, more accommodating shoe to allow room for your toes to move. This includes shorter heels, softer leather, wider toe boxes, and comfortable gym shoes. Using a pumice stone to thin any corns and calluses is also sometimes helpful. Cushions and various soft pads may provide relief from shoe pressure over the toes. Never use a “medicated” corn pad, since these contain a strong acid that won’t differentiate between the bad and good skin. This can lead to a chemical burn or deep open sore that can become infected.

If these conservative therapy options don’t alleviate the pain, or when you cannot find shoes that are comfortable enough, surgical correction should be discussed with our podiatrist. The longer the surgery is delayed, the greater the amount of deformity will develop – and the more complicated the surgery will be. Patients who have their symptomatic hammertoes and claw toes corrected earlier tend to have greater satisfaction after the procedure.

Surgery for Hammertoe & Claw Toe 

If you decide surgery is the way to go, our goal, our goal will be to restore normal alignment and function of the toe joint. If the toes are still flexible, the tight tendons can be released and the toe straightened through a small poke-hole in the skin. If the toe is rigid, the knuckle is straightened and returned to a normal alignment. A small pin is sometimes placed inside the bones to hold the toe in proper position while it heals. If the joint is destroyed from arthritis beyond repair, it may need to be replaced with an artificial joint. If a bone spur is the cause of the problem, it can be filed down through a small poke-hole in the skin.

Surgery Recovery Time

Following surgery, your foot will be bandaged and you will wear a post-operative shoe for one week. Athletic shoes may be worn after the first week. We advise against stringent exercise and prolonged standing for the first 6-8 weeks but agree that a home exercise program is important for regaining strength and flexibility in your toes. Consult with your surgeon as to when you may steadily resume activities and wear more fashionable shoes once the healing is complete.

WFAI COVID-19 Announcement

Update (4/8/2020):

We are requiring patients and anyone accompanying them to arrive for their appointment with a face covering, (such as a scarf, bandana, t-shirt, handmade mask or other) that covers their nose and mouth already in place.  If they or their guests arrive without one, they will be asked to return to their car to put one on or reschedule their appointment.  We are unable to issue surgical masks to patients at this time.  As such, beginning on the morning of April 8th, we will require patients and visitors to arrive for their appointment already wearing a face covering.
