With hammertoes, your toe is bent at the middle knuckle of the toe, while a claw toe means the toe is bent at both the middle knuckle and tip of the toe. A bone spur can happen anywhere on the toes but usually affects the 4th and 5th toes. Are Certain People More Prone to Hammertoe & Claw Toe?

The answer is yes. People born with a certain foot type are predisposed to hammertoes and claw toes, including those born with flat feet, high-arched feet, or really flexible feet. Other causes are extremely long toes, tendon imbalance, injury, rheumatoid arthritis, and neuromuscular disease. Over time, the mechanics of your foot and shoes increase the deformities. Women are more affected by these conditions than men, more than likely because of the types of shoe they wear and the use of tight stockings.

What Are the Symptoms of Hammertoe & Claw Toe?

With both these toe conditions, the severity of the deformity may not match the degree of pain. For example, a hammertoe or claw toe that looks severe may have no pain; while deformities that look mild may be incapacitating.

Pain and corns usually grow over the toes due to the friction of the shoe in that area. This rubbing of the shoe may cause a red, inflamed sac of tissue called bursitis. In the beginning, the deformities are flexible and can be treated easily. However, if left untreated, they can become rigid or stiff and cannot be straightened easily. Over time, pain may develop deep in the toe joints and even the ball of the foot, limiting walking, exercise or even just standing comfortably. Wearing high heels, pointed shoes, and tight stockings can worsen these symptoms even more.

A bunion deformity involving the big toes can also result in hammertoes and claw toes – or make them worse. When the big toes start to angle over, the big toe may cause increased pressure on the second toe or the second toe may be pushed upwards out of the way. 

What are the Treatments for Hammertoe & Claw Toe?

If your pain from hammertoes and claw toes is mild or moderate, the simplest solution to try is a wider, more accommodating shoe to allow room for the toes to move. This includes shorter heels, softer leather, wider toe boxes, and comfortable gym shoes. Using a pumice stone to thin any corns and calluses can also be helpful. Cushions and various soft pads may provide relief from shoe pressure over the toes. Never use a “medicated” corn pad, since these contain a strong acid that won’t differentiate between the bad and good skin. This can lead to a chemical burn or deep open sore that can become infected.

If these conservative therapy options don’t help ease the pain, or when you cannot find shoes that are comfortable enough, consult with our podiatrist about surgical correction. Sooner is better than later. Patients who have their symptomatic hammer toes and claw toes corrected earlier tend to have greater satisfaction after the procedure. The longer the surgery is delayed, the greater the amount of deformity will develop – and the more complicated the surgery becomes. 

What to Expect With Surgery 

If you opt for surgery, our goal will be to restore normal alignment and function of the toe joint. If the toes are still flexible, the tight tendons can be released and the toe straightened through a small poke-hole in the skin. If the toe is rigid, the knuckle is straightened and returned to a normal alignment. A small pin is sometimes placed inside the bones to hold the toe in proper position while it heals. If the joint is destroyed from arthritis beyond repair, it may need to be replaced with an artificial joint. If a bone spur is the cause of the problem, it can be filed down through a small poke-hole in the skin.

Recovering from Surgery

Once your surgery is complete, your foot is bandaged and you will wear a post-operative shoe for one week. Athletic shoes may be worn after the first week. We advise against rigorous exercise and prolonged standing for the first 6-8 weeks. However, we agree that a home exercise program is important to regain strength and flexibility in your toes. Consult with your surgeon as to when you may steadily resume activities and wear more fashionable shoes once the healing is complete.
